Mental health was never only a mind problem

If you feel unwell in the mind, is it only because a thought went wrong? A common picture of psychological work is complaint, a few consoling sentences, a change of angle. A good clinician behaves more like a detective: sleep, food, money, the room you live in, the parents still in the story. You do not live in a vacuum. What sits outside the psyche often decides whether anyone can recover inside it.


Health is not an isolated biological event. It is a long negotiation between a person and an environment — what is sometimes called a social-ecological model. Feeling is woven from history, money, support, and the neighbourhood underfoot. Anxiety and low mood in a context of poverty or injustice can be an accurate reading of the facts, not a private malfunction.


The pandemic made this cruelly clear. For people already short of resource, the threat to mental health was the missing meal and the unaffordable clinic. Asking someone in that position to think their way to calm is a kind of civilised arrogance. What is needed first is often material support, not an analysis of character.


A friend widowed, then unhoused, then sleepless, did not lift because someone listened well in the first months. The lift began after work returned, rent became possible, and sleep improved with movement. If the body is hungry, wandering, or spent, the soul will not be repaired by a change of attitude alone.


Lack of resource multiplies pressure. Unemployment, no medical cover, long discrimination — these sit outside willpower. Talking about an inner world without looking at the ground under the feet is incomplete. Communities need more than extra consulting rooms. They need shared nets a person at the edge can catch. Mental and physical health, and the social terms of a life, do not come apart.
